To Our Patients:
As this country and world battles the Coronavirus pandemic, we at Horizon Internal Medicine understand and care what a challenging time this is for our patients and people across the state, country and world. We would like to introduce a new program called “Horizon Cares.” We would like to help those we can by providing the following services at no cost to the patient:

Home visits for the elderly (>65 age), disabled, or handicapped patients.

Provide household supplies to those unable to leave your home or have difficulty in obtaining.

Provide food to those unable to leave your home or having difficulty in obtaining.

Go to the pharmacy for those unable to go or has a pharmacy that does not deliver with the exception of controlled substances.
